ACA Report Data

Description
  • Medical plan enrollment data necessary for 1094/1095 forms by calendar year.
  • Employees who are active at any time during the calendar year.
  • Data includes
    • Enrollment status, coverage tier, coverage start and end dates, and the total premium for the enrolled duration
    • Enrolled dependents with coverage start and end dates
    • Employment details, home address, and DOB 
  1. Select the plan year in the Year drop down field
  2. Click on Generate.
  3. In a few minutes, click Refresh.
  4. Click on the Report Link and the file will open in Excel.
