ACA Report Data
Description |
- Medical plan enrollment data necessary for 1094/1095 forms by calendar year.
- Employees who are active at any time during the calendar year.
- Data includes
- Enrollment status, coverage tier, coverage start and end dates, and the total premium for the enrolled duration
- Enrolled dependents with coverage start and end dates
- Employment details, home address, and DOB

- Select the plan year in the Year drop down field
- Click on Generate.
- In a few minutes, click Refresh.
- Click on the Report Link and the file will open in Excel.
